Our Eagle and Meridian office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help. For your convenience, our front office is available to take dentist appointments and answer questions Monday – Thursday from 8:00 a.m. – 5:00 p.m.
Should you decide to join us as one of our valued dental patients, you will find new dental patient forms located under the ‘New Patients’ tab of this site. You may choose to print and complete these forms prior to coming to your dentist appointment, or you may arrive about 15 minutes prior to your scheduled dentist appointment time and complete this information in the comfort of our office.

Financial Information
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For dentist patients who require major work, a complete payment plan is designed with an appropriate payment schedule via CareCredit. Forms of payment accepted by the office are check, cash, or any major credit card.

Cancellation Information
If you are unable to keep your dentist appointment, we ask that you kindly provide our Eagle office with 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
